How to Get Your Replacement Passport

Step 1 – Reserve Your Submission Place your order online to secure your passport processing date.

Step 2 – Download Your Required Checklist Do not visit an acceptance facility without our official instructions. Our printable checklist includes all required forms (DS-11 and DS-64) and the exact steps to avoid common processing delays. 

Step 3 – In-Person Identity Verification Take your completed documents to a local Passport Acceptance Facility (Post Office or County Clerk). An official agent will verify your identity, witness your signature, and seal all your documents into an official “Adjudication Envelope.”

Gather these items before your post office appointment for a new passport.

1) Photocopy of a Valid Drivers' License or State ID Front and Back

  • If your ID was issued in the last 6 months or is expired, provide at least two additional IDs.

2) Evidence of Citizenship - Must provide 1 of the following

  • Fully valid, undamaged U.S. Passport (can be expired)
  • Original or certified copy of your U.S. birth certificate
  • Certificate of Naturalization

3) Passport Fees - (Check or Money order made payable to the U.S. Department of State)

  • $212.05 payable to the U.S. Department of State for a new 10-year passport book
  • $242.05 payable to the U.S Department of State for passport book and passport card
  • The fee includes 1-2 shipping ($22.05) directly from the Philadelphia Passport Agency

4) Passport Photos - (CVS, Walgreens, Staples, Post office, or Clerk's office)

  • No glasses
  • Taken within the last 6 months
  • In front of plain white or off-white background
  • Neutral facial expression with both eyes open
  • Do not wear a hat or head covering that obscures the head or hairline

5) Travel Itinerary( Print 2 copies )

  • Your name must be on travel documents.
  • Print your confirmation flight itinerary, emailed to you by your travel provider.
  • Hotel reservations, Airbnb, or Print Driving Itinerary and complete (only for driving across the border) 
  • Business travelers’ letter from the employer on company letterhead stating emergency passport needs.

Step 2: In-Person Identity Verification (Lost Passport)

  • Visit an Acceptance Facility: Take your completed DS-11 application, DS-64 (Statement Regarding Lost or Stolen Passport), and all required documents to a local Post Office or County Clerk.

  • Witnessed Signature: An official agent will verify your identity and witness you signing the application.

  • The “Sealed” Envelope: The agent will place all your documents—including the DS-64—into a secure, official government envelope and seal it.

    • CRITICAL: Once handed to you, do not open this envelope. If the seal is broken, the passport agency will reject your application.

  • Send to Us: Immediately ship or hand-deliver this sealed envelope to Prompt Passport Services so our couriers can hand-carry it to the agency for expedited processing.
